Vaccination Requirements

As a new student at °ÄÃÅÁùºÏ²Ê¿ª½±¼Ç¼×ÊÁÏ, you are required to submit your vaccination history to health services. To keep our campus healthy, certain vaccinations are required.

How to submit your vaccination history

You can track them down through:

  • Your family physician
  • Your high school or previous college/university
  • Your branch of military service

To submit your vaccination history:

  • Mail or fax an official copy of your vaccination records to Health Services

Complete your vaccinations

We encourage you to meet with your primary provider to complete vaccinations prior to coming to °ÄÃÅÁùºÏ²Ê¿ª½±¼Ç¼×ÊÁÏ to receive the best financial coverage. If you would like to have your vaccines or blood titers completed at Health Services, please call us at (616) 526-6187 to schedule an appointment. We will bill your insurance for you, but cannot guarantee reimbursement. We strongly recommend contacting your insurance company prior to your visit here to confirm your beneifts and coverage at the Health Center. Bring your insurance card to every appointment.

International Students: Please wait to fulfill any requirements for vaccinations or tuberculosis testing until you arrive at °ÄÃÅÁùºÏ²Ê¿ª½±¼Ç¼×ÊÁÏ. These services are covered in full by Calvin’s Student Health Insurance Plan.

Deadlines

All requirements must be met by the deadline specified in new student correspondence from Health Services. Ideally, these are all met by the start of your incoming semester. Health Services does allow for a 30-day grace period, into the semester, before any fees or consequences are assessed. If you are not compliant by the deadline, you will be charged a $50, non-refundable, fee and your registration account will be put on hold. If a registration hold is placed, all requirements must be completely satisfied before this can be lifted.

Tuberculosis screening

If you fall into a high-risk group for tuberculosis, you must have a tuberculosis test completed. You may do this with your primary care provider, or at health services.

Request for Immunization Records Policy

Immunization Records are kept for 10 years from the date of graduation or departure from °ÄÃÅÁùºÏ²Ê¿ª½±¼Ç¼×ÊÁÏ. Completion of the Immunization Request Form is required for each request. Upon receipt of the completed form, we will process the request within 2–3 business days. The completed form may be faxed to (616) 469-1398, scanned and emailed to health@calvin.edu, or dropped off to Health Services for processing.

For vaccines administered in the state of Michigan, individuals are able to view/print off their immunization history through the State of Michigan via the Michigan Care Improvement Registry (MCIR). Please visit to log-in and access your information.
